Players Registration Forms1. Harbord Seasiders United (HSUFC) Rego form
All existing & new players to HSUFC need to fill in the HSU Rego form.2. Manly Warringah Football Association (MWFA) formThis form is required for U/12 players upwards who have not played in Manly Warringah before.3. Date of Birth (DOB) Statutory Declaration
Form required for Juniors U10 (including U9 if dual registered) and upwards who never played in MWFA before and don’t have Proof of age4.A. Football Federation of Australia (FFA) form
All new Players are required to complete an FFA form to receive a dedicated FFA Player Registration Number.
Existing players will have a pre-printed form with the FFA Number but needs to be signed by each player.

4.B. Football Federation of Australia (FFA) Supplement Form
Only Needed : 1) Register as a Match Official 2) Transfer or be loaned to another Club within Australia as a Professional Player 3) Register for another form of football, as permitted in Section 4.9(b) of the National Registration Regulations 4) Register for another club as an Amateur player after de-registering from his/her original club.

5.A. International Transfer Certificate – USA (ITC) 
Only if played in the USA in 2011 (no matter what level) this takes a minimum 30 days to get.

5.B. International Transfer Certificate – Non USA (ITC) 
Only if played overses (other than the USA) in 2011 (no matter what level) this takes a minimum 30 days to get.

All senior team managers, coaches and players MUST attend rego days to fill in forms and get a digtal photo taken to be printed on the registration cards.

This is a Document which defines the scheduled weeks for games for all number of teams/divisions for the season to indicate the dates for the Semi/Finals and Wash Out Weekends.

This program is part of the Football NSW Limited Risk Management and Player Protection Plan. It is designed to supplement Loss of Income and Non-Medicare expenses but BY LAW THIS PROGRAM IS PREVENTED FROM ASSISTING WITH ANY MEDICARE ITEMS. The benefits are limited. Note it is not a Health Insurance Scheme or Workers Compensation.

All Managers and Coaches in all age divisions need to sign/return this form. You are giving consent to the Comission for Children and Young People to check your criminal records.

Dear Parents

A note on HSU Registration Fees

 Firstly, you may have observed that some other clubs appear to charge less for juniors than your local club.

What is not necessarily clear is that, on top of their up front fee, shorts & socks will also need to be purchased for anywhere up to $30 per child.

If you add this additional cost you will see that HSU’s fee structure is generally cheaper with NO hidden costs.

Secondly, when deciding on a club for your child, please ask what training facilities, equipment and support your child’s team coach will receive. Once again, we believe that HSU’s commitment to ensuring our fees are returned to the players will make you very pleased with the value for money you receive.

Finally, if you have more than one player registered please remember that at HSU you can claim a $20 per additional player family discount.
